The constant tension clamps are used on coolant hoses on MANY cars, primarily American and Asian. European cars are pretty much the only ones I work on that don't use many of them.

More than likely a junk cap, I wouldn't use those Gates caps. Only use OEM expansion tank caps.

The stock caps on these Volvos are just fine, I haven't heard of any issues resulting from the pressure. I would keep the OE 1.5 bar cap.

Junkyards, eBay, etc. Anywhere you can find used parts, really. I always try to grab the later (2013 or so) date code units from the yards, as those as the best solution IMO (as Abscate said).

It's hard to tell if that's corrosion or coolant residue on the pipes, but if it's coolant it may be time to replace. Otherwise, the OE hoses do last a really long time.
