Low coolant light, poor mileage in town

2000 V70 XC I have an intermittent problem with the COOLANT light illuminating. I check the level and it is usually between MIN and MAX(ocassionally add Anti freeze). The light might stay on for a couple of trips and then extinguish. My around town mileage is also very poor, 13 mpg, and the check engine light occasionaly illuminates. I reset the check engine light by disconnecting the battery then all is fine for couple of days.
The car nearly overheated when I bought it last year and a new thermostat fixed that problem.
The last related item, the cooling fan almost always stays on for 5 to 10 min after I get home regardless of letting it cool down.
Any suggestions would be greatly appreciated

The first thing to do would be to get the code/s read the next time the check engine light comes on. AutoZone or PepBoys can read them. Then you can go about fixing the problem.

Sounds like the car may be overheating and boiling away the coolant. You don't want to let that continue.
