Anybody know if this eventually happens to all of our throttles or is it more of a rare occurence?

Rare. It's something for tuners to check next time you're in there (it's easy), but I've never heard of it happening here on the MVS Volvo boards.
The more you mess with your engine, the more likely it is to happen. If the throttle/cable has never been touched on, I'd be surprised if it was slack.
