Heavy Shudder on start-up

Hi! I've been lurking here a long time, but just now created an account :D

I have a 2002 V70 XC with 132,000 miles on it. It is running fine overall, but maybe 1 in 10-15 times starting the car it has a heavy shudder on start-up (shakes the whole car noticably) that lasts about 30 seconds. I always let it resolve before putting the car into gear or hitting the gas. It runs like a top down the road and has plenty of power - no noticable problems any other time. The shudder seems to originate in the engine compartment. It doesn't seem to matter if it's a cold or warm start either.

Any ideas on what this could be?

I have experience the shake from hell, coming from the engine compartment. First simple step would be to check the battery. I had the original one which lasted 11 years. It is almost a year that i have not had any.

I popped off the cover and took a look today - the battery is less than 2 years old and tested fine, so I don't think that would be it.

I would search the forum for "cold start".
I recall it was vjaneczko, who said it is better to find 5-7 symptoms that best describe your situation, and start with simple with no cost first. It made sense to me.
