1998 Volvo S70 GLT Hesitation at 2000rpms

My 1998 S70 GLT has a hesatation around 2000rpms, at higher RPMs (2500+) and MPH (65+) it is fine. It was doing this every once in a while but after CrankCase Vent system being replaced, it has become worse. It now hesitates almost every time at 2000rpms and drops to 1500-1700 rpms (the car shakes) and then goes back to 2000rpms.

I would start by checking for vacuum leaks and if none found, how long has it been since its had a good ol fashioned tune up ?
