Transmission Service Urgent Gear 5 Incorrect Ratio

The wife tells me last night that the display read transmission service urgent in her 2001 V70 XC and that she did notice a surge in RPM's when accelerating. When she informed me last week that the CEL came on, I scanned and found the following codes P0030 (HO2S Heater Control Bank 1 Sensor 1) and a generic P0168 code. I cleared codes and researched to find that the P0030 indicates a possible O2 sensor failure. As I didn't think this was a "10" on the scale of seriousness and safety, I told her she could drive locally.

I just went out to drive the vehicle and determine what was going on. I Checked transmission fluid and it was clean and full. I drove vehicle no more than 1/2 a mile before I noticed what she was referring to with regards to the slipping. I drove another 1/2 mile to get back home and went to back into parking space and got no response in reverse. I then placed in Drive and got no response there either. Tranny is shot and non responsive. I pushed into parking space, turned off and grabbed scan tool. New Code appears in addition to the ones above, P0735 stating Gear 5 incorrect Ratio.

I recently replaced the right front drive axle and hub bearing assembly, however, not at the same time. I replaced the axle and found the noise / vibration was still there, so purchased the hub assembly a couple of weeks later and had to remove the axle in order to replace the assembly. I felt I put everything back together fine but wondering if I could have done something to facilitate this transmission issue. I should note that the axle was a product of China and not a OEM part, and cost around $100.00 shipped to me but the exact same part visually as the OEM I removed and replaced this with.

Hope the post isn't too long, wanted to provide as much info as possible. Look forward to your comments / response.

BTW- This vehicle has 148k miles on the odometer and I purchased used with 100k 4 years ago. I never changed the fluid and not sure if it ever was changed.

Hi. Did you figure out exactly what was going on with Transmission in the end ?.... I just bought a car V70 Turbo 2000 that has done exactly the same thing. Ive been told that if I go back and try car again and it does go for a minute or so then sit for 30 try again. and it goes for a few minutes. It has debris in the filter from parts that have worn or ?. I'm told when it sits for a period this falls from filter only to be sucked up and block it again. Tried it and yes this is happening
Car had a new Valve Body put in a year ago shown on receipts for previous owner.. I need to get it rebuild in short..

The codes that came up are and readable on my basic code reader
P0735 - Gear 5 incorrect ratio...
P0740 - Torque converter clutch solenoid circuit open...
P0798 - Pressure control solenoid C electrical
P1618 - ?
P1171 - ?
