Starting from the beginning, even if late, I don't see any diagnostic being done - perhaps they did and it's not mentioned here. Why would one replace expensive o2 sensors w/out testing first? Especially when it's so easy with any average code scanner. And the cat efficiency... Was there testing done for air leaks that may fool the rear o2 into fast switching... did not see that either. But at this point I'd do the spacer like you said, it's been done on the S60 forum and perhaps others, and go after the misfires. With the misfires.. there is freeze data stored in computer that shows status of engine parameters at fault time. I'd start there as you may have a lean condition that may cauze misfires. It's sad that with all this testing equipment available owners still have to pay for repair jobs that don't actually repair anything.
