I don't know if this is just being nitpicky, or indicative of an actual issue.
Under mild/light acceleration, the car "rumbles". I don't know how else to describe the noise.
It is still accelerating, perhaps not as quickly as it should, but then again, it has 263k miles on it.
If I give it 75%-100%, it moves with no hesitation and sounds normal.
Vacuum leak?

What error code do you have? My car has over 300,000 miles on it and pulls like a train.
