I think I have the PCV off my 99 base parts car, it's broken but might help you sort out the pieces for a repair strategy.
I'll check the parts bin later today and see what I have

Well its a 10mm ID Plastic pipe running inside that foam exterior. The rubber ends have 13 mm ID on the PCV can end, and ??on the other end.
The thin vacuum line pulls off the elbow and has a 6mm ID on the other end. I doubt it would screw things up too much if you used a 10mm ID hose and maybe re-sued the end with the vacuum attach point or I can send you mine. It does look like new.

Is the large elbow that also connects the small vacuum line a separate part? You make a good point that I may be able to reuse that larger hose, and just replace the smaller one.

It's all one piece. The small vacuum line attaches to the elbow with some kind of office inside.
It's a freebie, I would probably cut it down to fit in a small box though.
I took photos before I travelled but they didn't stream this morning so give me a day to sort out

Two elbows cracked within months and it gave me erratic RPM issues at idle and when coasting to a redlight in neutral. Do not buy. Now i have to source a new one and remove my intake manifold to redo this job.

I actually made mine out of copper pipe, elbow fittings and short sections of rubber hose from home depot.
All the copper was soldered after dry fitted.
if if I have to do it again, I'll just use rubber hose.
This was one of those projects that started with "This will be really cool" that turned into a "Why didn't I just do this normally???"
