Have you scanned for codes? There are some codes that will set even when the CEL isn't lit and I think weak or intermittent signals from the camshaft and crankshaft position sensors are just such codes.

I am sorry but I don't recognize your name on here and if you have posted on this issue before. So please help us to help you so we are not just (hopefully) stabbing in the dark or sending you in the wrong direction. I don't know your knowledge/experience on cars or particularly Volvos.
It would be helpful if you can give use the following information:
1) Have all the parts been changed to try to fix this problem or has the problem got worse or started since changing parts?
2) What parts have you swapped out? Please remember to give brands f the new parts as these Volvos are particular on certain brands of parts in some parts of the system.
3) You say the fuel pressure is ok, but what did you get and how was it tested? Again this can help us as these cars are now 20 years old and some parts like fuel pressure regulators can fail causing longer starts while the vehicle tries to get going.
4) Have you cleaned or repaired/replaced any parts of the fueling system like the MAF? Again brand is important.
The fact the car runs well once started is a good thing, but hard starts are not unknown but then there are some fixes too.
Neil.

Fuel pump relay or main fuel injection relay on fan shroud(the grey one) could be the culprits.
I would say intermittent chipped key transponder but you guys didn't had them untill '99...but could be also immobiliser problem.
