Need to replace this leaking HOSE, and yet cannot identify it. Front end goes up into the same port as upper intercooler hose, then burrows inches right of intake and beneath throttle body. ID is about 1", and stretched is about 10-12" long.

Looks like it pulled from the oil separator box.
Its under the manifold so will require you to remove intake manifold.
Its part of the pcv system that is likely clogged.
Check ptc nipple at air pipe that goes to turbo down low past maf it has an electrical connector and 2 hoses attached to it. Takes ventilation from engine separates the oil and air puts air in intake and oil back into block. The ptc generally clogs and allows air pressure to build and blow the weakest link in your case looks like oketier clamp that held hose to oil separator box.

Hose between the Charge Air Cooler (CAC) and the Idle Air Control (IAC) Valve, which is built into the intake manifold.
