I’m going to push you away from the “ junk in tank diagnosis”. You don’t get crap in fuel anymore unless someone hates you and puts it in your tank directly, likewise for plugged fuel filters.

"Old junk" hanging around in a 19-year-old car and finally causing problems? Does the inlet really have an impeller that can fail? Is it powered? I have everything needed to disassemble/reassemble the entire fuel pump unit (but not replacement parts). Replacement won't answer the question without a diagnosis.
I could drive it two floors down into the parking lot at work hoping it will fail.

I also think I read that the PEM was introduced to the XC70 sometime during the 2004 model year. Maybe my 2004 XC70 is pre-PEM? Would a VIN lookup provide that info? How about a scan tool?

I don't remember finding any information via the VIN or using a scan tool about presence or absence of the PEM in my 2004 XC70. I never found one by looking in all the known locations as described earlier in this thread. Vida/DiCE seems like it might provide info about the presence/absence of a PEM. I have Vida but not DiCE so I can't try this. Does someone know for certain? As in have you detected lack (especially) and/or presence of a PEM this way?

I believe this is correct. Some 2004 models do not have PEM while some later models do. In my case, thhe pump itself appeared to be fine but the fuel pickup/impeller appeared to have failed. I can't say of this was caused by wear or contamination in the fuel but it was definitely not free to spin as the new one was.

i ran my old volvo on fumes too often and burned the pump up.
