Trouble code 2177 & 2188 at same time.

My 03 V70 is throwing 2 trouble codes that seem to be contradictory. Engine too lean at start up and engine too rich at start up. Car runs great 90% of the time. The car has 205,000+ miles on it. I have replaced vacuum lines, injectors, plugs, fuel filter, O2 sensors, MAF sensor,throttle body and the accelerator pedal. Throttle body and pedal were replaced from a different code last year. I'm now thinking about replacing the intake manifold gasket. Any help is appreciated as this is my daughter's car.

it would be a candidate for smoke testing the system, vacuum leaks are hard to find visually or by listening.
smoke will expose the leak. i wouldnt pull the manifold without verification with smoke.

i bought a cheap transfer pump and stuck a cigarette in the inlet to pump smoke into the intake systen.

Smoke tested and found no vacuum leaks. Not sure what to do now.

Coolant temperature sensor? Engine uses this to alter fuel settings when cold.

Engine temperature sensor. I can't recall if the 03 has one on the intercooler pipe from the intercooler to the throttle body.

its next to the thermostat, requires 18mm deep thin walled socket, vida will show if its reporting incorrect temp.
I replaced mine twice, both sensors were no good brand new, I reinstalled the old one (thermostat was stuck open).
verified by shooting the thermostat housing with an infra red gun and comparing to what the ecm was showing.
i wouldnt buy any parts until verified by diagnostics.

what are the symptoms when it doesn't run right.?
Do you have vida, did you buy oem parts ?

Sometimes it chugs and sputters on start up, other times it revs to around 4500 rpm. Most of the time the cars runs great with no symptoms except for the check engine light.

I would monitor throttle module with vida.
Vida can monitor 6 different parameters simultaneously, throttle body would be on the list
4K rpm is like WOT. I can't see anything else causing that high or rpm than WOT.
It might be sticky.
