A couple years ago the upper torque mount hardened and started clunking, so I picked up an IPD urethane one (having had good luck with the urethane radius arm cone bushings and accessory bushings on the 740). Been through a couple as they have been crumbling to dust and chunks after about 2 years. Am I doing something wrong with the install (support bolted to engine block, insert put in, through bolt tightened), or is there something about these that just doesn't work well in that position?

Nope you are not doing anything wrong. I would suggest they are just crap. Get a rubber filled one.
