Was working intermittently and erratically, then stopped working completely.

In addition the the fan control module, there are two relays and the motor that can cause it to intermittent.
I would start with the motor.
Pull the loom off the motor and hook both the Positive and Negative up direct.
Tap on the motor to see if it will intermittent.
The modulating fan speed, intermittence and erratic are what makes me suspicious of the fan motor.
Those three are symptoms of worn brushes.

Mine has been erratic for the past 5 years (at least!).
I changed the resistor 2-3 years ago and it didn't make any difference.
I generally don't use the fan anyway. Rarely use A/C or need any boost (blower fan) in my daily commute.
Still, it would be nice to have it working properly so I'm going to follow up again with this new information.
