My wife’s 2010 XC90 (150k miles) a/c only blows cold when she’s on the highway. Blows hot air all other times. The condenser fan in the engine compartment comes on and off appropriately with the A/C being turned on or off. I can also hear the compressor engaging / disengaging.
Also - maybe related or not - air (of any temp) will only blow thru the defrost if the car gets up to 45-50 MPH. Otherwise it blows through the middle vents.
Any ideas?

Is your blower fan working? I would dismantle the passenger side and pull out the fan to inspect. May also be able to bench test using a 12v source. Also dampers could be an issue but it sounds like highway speed air pressure is able to make it through. Sometimes I've heard the fan control knob can be bad too.

Any ideas?
Will you get cold air each time the condenser fan and compressor comes on? If so you can rule that portion out for now and focus on the controls and related dampers.
