If you are getting misfire codes or even the dreaded random misfire code and have replaced coil packs and plugs to no avail here is what I can offer.
IF the misfire shows up during a cold start up or a cold idle, you can hear it at the exhaust, sort of a puff puff, then you may have a bad injector. I chased a Cyl #1 misfire for 7 months and 30,000 km before I thought to swap injectors and the misfire moved with the injector.
There is a code for misfire at start up but the typical code reader will not see it, it will give you the cylinder misfire only. You need the Volvo software to read just when the misfire occurs and I did not have that.
Just to be really safe, since I have just passed 300,000 Km, I did new Volvo plugs, new coil packs and new injectors. The idle is very smooth now and the car seems to run much better wide open. Parts came from IPD at less than 1/2 what Volvo wanted.

There is also the fuel rail pressure sensor which if bad will cause the same problem and leave the random misfire code. The check engine light blinked at start up but once the fuel pressure built up it was fine. The mechanic that works on mine has just replaced two on later model R's in the last three weeks.
