How does it work, when does the little flap close the air flow from the exhaust and lets the outside air in?

Thermostat opens a vacuum port and the vacuum moves the door. The changeover to exhaust proximity air heating is at 5-8 C for most systems.

That is for your S60. Some use direct thermostat motion to move the door but it is spring closed and t-stat has to expand to push door open. The direct version is very much prone to quit working and leave you with heated air that hurts warm weather performance and can kill the MAF.

I looked at it and there is a thermostat, a spring, and also a small vacuum hose from the top of the air box. If I understand well, at temps above 5C the door for the exhaust air should remain closed?

I removed this thing, the air box thermostat - not an easy job as the whole air filter box has to come out. Thrown it in hot water and in the freezer and it doesn't seem to move much. Time for a new one.

does that have a hose attachment on the back side?

Yes, there is a thin hose attached to the cover that keeps the thing in the picture in place

Removed the stat. It expands 1/8" when in hot water

Alldata had no info on this, so great that this is in the reference section

Got trough this, permanently removed the thermostat.

When the s-stat fails, the engine gets continuously warm air from the exhaust, which doesn't help combustion. It might be a reason for low mpg so this fix hopefully helps increasing the mpg
