You need to start by pulling the codes. An AutoZone scanner will suffice. Post the results back here.

Very loud does sound like its bearings are beginning to fail. But find out what that CEL is about before you go buying parts.

I've noticed a peculiarity in my 1999 that when my MAF is disconnected the fan runs full time but get codes first, as the man says, diagnosing these cars by symptom isn't productive

If the fan itself is OK and you can turn on or off via the diagnostic scanner, then you have a problem is likely with air conditioning. Check the pressure in the air conditioning system. In my car, I temporarily emulated the signal of pressure sensor for conditioners system and my fan was not working.

You may need to replace the fan which is very easy.
