I would place the MAF in serious consideration. A disconnected MAF is supposed to idle, and not be able to get around the block. If this were a turbo, I would have said that one of the intercooler hoses was loose or broken. For non-turbo, check the air filter box to throttle body tube very carefully...

On the bench testing, have you tried cold soaking it in the freezer for an hour?

I think a combo of what Rick and Steve have suggested can work using self threading bolts/screws with large washers, drilling a couple of holes and first heating the plastic before putting the screws in.
