It is the bearing failing as wcm says. Time for a new fan motor. Removing it and spraying the bearings with oil may help short term until you get a new fan motor.

Did you check the coil on the relay? The relay won't switch if the coil is open.

IMHO, the SAS is more trouble than it is worth. Its contribution to the environment is just as questionable. Look up SAS delete.
