The clunk can be many things. The slipping though is pretty diagnostic. I have not had a trans go out on me but have seen it posted over and over again: on these cars, the first sign of the transmission going out is slipping in reverse.
I wold try one more fluid change, i.e. another 3.75 quarts. If that doesn't help with the slipping then it will probably need a trans. Many people just keep driving it and the problem remains only with reverse, other times the whole trans soon gives up.
The turbo is pretty easy to remove in the driveway, doesn't take long at all. A couple bolts come out from the bottom (I put the car on ramps) then I sort of lay on the engine for the last bit and pulling it up and out. Most of the junkyard turbos will be just fine, check the shaft for play first if you go that route. The other option is to rebuild it yourself (I've never tried that but many here have) or pay about $400 for a rebuild; G-pop shop is e great source for that (http://gpopshop.com/).
Replacing the trans: I have read of pulling the trans out the bottom, you will need to do some searching. Or pull the engine and trans out the top together, and give it a full overhaul (timing belt, seals, transmission, turbo cooling lines, turbo) all at once.
