Could anyone recommend an OBD-II scan tool that can read the actual mileage of the vehicle off the ECU? My odometer gear broke and I have the replacement gear, but even though my title is branded "not actual mileage", I'd like to get as close to the real mileage as possible when I do the replacement.

I deal with one of the suppliers from the Alibaba site for business purposes and while they are now 13 transactions into dealing with them I would NEVER do so without an English speaking contact there.
If they can't answer your questions in any great depth you will more than likely end up with the wrong product or one that doesn't suit your needs and you can bet they won't refund you if you choose the wrong product.
Alibaba recently fudged one of my orders because I mistakenly paid for two orders with one transfer and it took four weeks and 10 emails to sort it out, and that's when the email system WAS WORKING. With Alibaba items you are paying ALIBABA directly then they notify the seller of cleared payment
Does the seller have feedback? The Alibaba system allows this. Watch out for the payment methods as if you International Transfer there are large fees from your bank. The site recently dropped Paypal as a payment method so credit card or Western Union are other options.

Amazon currently has the Autel MS300 at less than $22 and the MS509 for less than $80.
I think those are pretty good prices. The downside is the non-existant customer support
from Autel. But reviews of the 2 scanners are good.

I think my VIDA DICE was about $120 from Amazon. It is a Chinese clone, but that did not deter me. As long as you have the right version of Windows 7 and IE, and if you feel comfortable with VMware it i too easy to use. It will do everything the dealer can do except calibrate the soft top. I also have an elm unit that works from Bluetooth along with Torque from the aop store, Both will do real time data logging. Dice also gives step by step instructions on repair diagnoses as well as diagrams and part numbers.

Windows 7 w/ .NET 3.5 if I remember correctly - check the VIDA sub forum as I think I put a post there on it.
Suggest 4 gigs of ram though I was able to run it on two.
I use an old Dell Latitude D630 to run it. Buy a car charger cable/adapter and you can run it for hours on end for the really sticky problems. They can be found cheaply. Spend a couple of bucks on a ~80-120GB SSD to make it a tad more durable for those of us that don't drive on smooth roads.
