Which sensor is the bank 2 sensor and how is it replaced?

Bank 2 is the rear sensor. You unscrew it, unclip the connector, put a new one in and reconnect. The wire is in the way so there is a special wrench that is open ended (auto parts stores sell them). Sometimes they are corroded on plus the constant heat makes them sort of fused into place so plenty of PB Blaster the night before helps it come out.

However, I will say to check your vacuum hoses, MAF sensor and other air/fuel mixture related items under the hood. Although it's unlikely, unmetered air getting into the intake and/or exhaust can set off 02 codes when they are otherwise good.
