Okay, this suggestion is going to sound a little out there.
Put all four corners up on blocks, disable traction control, and see if you can reproduce the problem without actually moving the vehicle.
This will narrow down whether it's a drivetrain problem or a suspension problem. It could indeed have something to do with your bad strut bearing, or a bad control arm bushing, or subframe bushing, or something like that.
EDIT: I didn't notice the additional pages before I posted this, and I see you did do some suspension work. I still think it's a good idea to try this as a diagnostic.

Some update:
The transmission has been performing in the same way, a small bump between 2-3 and sometime a clunk when put into reverse. My mechanic thinks that bump is so small it's nothing to worry about. Since the performance has not deteriorated at all I'll leave it as is.
So I'm inclined to believe the transmission is ok. Will probably going to tackle the subframe bushing sometime in the future. Will see if it solves the problem.

I quickly scanned the thread to see if you have checked the engine mounts or replaced any of them and didn't see it mentioned.
I just helped a friend's son on his 06 S60 AWD this weekend as he had some vibration and clunks. I had him order a set of engine mounts and it was a good call as the lower torque torque mount had failed but more concerning was the mount under the crank pulley was bad too. Upper was not too bad but replaced anyway. We did not have time to replace the 2 cone mounts. They still look good so can wait a few months until I hopefully get my 2 post hoist to give much better access to those 2.
The difference on starting the car and revving it made a huge difference in that the motor barely moved and no clunk on starting.
If the crank mount or torque mounts are bad you will get a vibration or clunk on engaging gears and in particular going for forward to reverse as the engine will move!

What ATF did you use? A lot/most of JWS3309-compatible fluids are low on friction modifiers, our Volvo-modded transmission shifts harsher than it does w/ original 1161540/1161640 ATF.
I soaked frictions in 1161640 while rebuilding the transmission, but filled it with Toyota T-IV ATF. It shifted good for maybe 3 days than the shifts quality degraded noticeably. Cured to acceptable shifts with 2 oz of Lubegard HFM, but will be filling with Volvo ATF the next time.
