Rattling sound on T4 drivebelt area

Hi. There is a problem on my T4 for quite some time now wich I was wondering if some of u can help me with. When the engine is hot and u blip the throttle to around 1500rpm's there is a strange rough rattling sound as soon as the revs drop down again to 800rpm's. I took the car to a Volvo mechanic 3months back and he said it was the timing belt tensioner.

So he took the timing belt off and inspected the tensioner pulley but it felt still very good, not rough or anything..dont know if he checked the waterpump though. The aux belt tensioner pulley was worn though so he changed it and the sound went away for 2days and then came back again. The car has got 89 000miles (143 000km's) may it be the altenator bearings or the waterpump?or idler pulley even? Even when the car is cold and idling u can hear a rough sound near the aux belt area. Any help will be much appreciated :) Thanx
