Paul, I checked the PCV all bolts are tight. If there was an exhaust leak I should be able to hear it (at least exhaust leaks I’ve seen on past car I was able to hear) I would assume. Please correct me if this though is not correct.
I have 2 O2 sensors just out of the back of the engine on the 3.2l I could change both but before I do I want to get your take on the thought I had about the exhaust leak. I did change the MAF sensor when the issue started.
Can the P0171&P0174 cause the car to be hard to start till after the initial start has been achieved?

Paul, as a test I unplugged the upstream o2 sensors one at a time and then simultaneously to see if a code would trip. I also did this for the fuel rail pressure sensor and MAF sensor. All scenarios caused a code. With that said then the ECU is communicating with each item and they are functioning. Currently I have no codes just hard to start on initial start (after it has sat for a period of time 1.5+ hours)after that it starts fine. The car runs good when running also (no performance issues that I can see).

Quick update my XC90 has been in the stealership for 2 weeks now while I’ve driven a loaner. They are still trying to figure out what is causing the issues with the car. They have determined that the battery, starter, injectors, MAF, O2, and fuel pressure sensors are all good. I will update as things progress.

What is the maintenance history? Have you owned it since new or did you buy it used?
Was the fuel filter changed only once, and perhaps could have been stressing the fuel pump for quite a while?
Do you notice any oil leaks? A leaking seal could cause a lean condition.
Have you confirmed the intake plastic tubes are sealed and don't have any cracks? Has it been smoke tested?
Have you ensured the gasket on the oil cap is sealing? This cheap gasket can be replaced in a few seconds.
There could be a different issue and you might have to confirm when it pops up...such as don't completely start the vehicle if it's struggling from cold. Then probe power and signal for the fuel pump control module (PEM). There is one case where there was a hairline crack, IIRC, in the board solder. Not a normal nor usual occurrence for the 3.2. Confirm the part number with the one you have if you choose to replace it with either a new or used one.

Yesterday I went and picked up my car. Stealership is an understatement. They wanted to charge me 3 diagnostic fees one for the CPU and electrical, one for the fuel pump, and one for the injectors. I fought that as I only brought it in for the initial complaint and did not add extra work for them to look at after drop off(no change order).
So after all that hunting the slow to start on initial start up was the fuel pressure was bleeding down over time caused by the injectors (that I just purchased from FCP) leaking per the stealership(we will see if they are right when I change them Monday evening and will update). They are under warranty and are already on their way here for replacement.
As for the misfire the stealership said the injectors should have coded a misfire code (don’t understand why it didn’t then since they say injectors are leaking) maybe someone can explain this part to me because it doesn’t add up to their diagnosis to me. The only reason it had a misfire code at my house was the carburetor cleaner sprayed on the injector with engine running was sucked in due to a bad o-ring. I’m still thinking fuel pump because per their diagnostic and their statement a bad injector causes misfire code which I do not have and the say fuel pressure is leaking down over time and there are no fuel leaks.

Did the stealership warranty the MAF and PCV parts?
Keeping an eye on the fuel system after replacing the injectors again sounds like the prudent next step. At this point, it does sound like you are chasing two separate issues.

Yes the stealership honored the warranty for the MAF and the PCV.
I saw a question about maintenance and felt I should address it. I do all my own maintenance usually if not, it goes to the dealer (only if I feel as if I’m chasing a gremlin basically). I do my maintenance per the owners manual. I did purchase the car new in December 2006 it even came with a big red bow since it was my wife’s Christmas present. Until recently all parts installed on the car were dealer purchases the injectors are the first items installed that weren’t purchased from the dealer.

Fixed! I know I said I would change the injectors on Monday but FCP wanted to argue over if the injectors should be warrantied even with a dealer diagnosis in writing provided to them. When this started I told FCP that they had sent faulty injectors but they insisted on a diagnosis from a reputable shop (apparently they don’t think the Volvo dealer is one of them). With that said the injectors arrived yesterday and were installed last night. I started and drove the car multiple times no issues found. The real test is what will it do after sitting all night. This morning I got up and took the dog to the park and would you look at that the car started like new no slow to start issues. So there you have it faulty parts supplied by FCP Euro. Sometimes I wonder if I should have bought these parts from autozone at least they don’t give pushback when trying to warranty out a recently purchased part and I only trust autozone for oil.
