Mine vibrates a little more in drive, esp with the wheels turned pretty far; if I'm sitting in a drive thru or something I'll knock it into neutral and it makes less noise. It shifts and idles fine otherwise. I'm thinking I need to change my ATF again as well (I did it less than 2 years ago, and it's starting to get brown again...sigh) and hopefully that will make a difference. I used to have a poly top engine mount in my car, and HATED it between 1000-2000 rpms; I went back to the OEM one. I can't really think of any reason why your car would only be loud intermittently though.

How does one replace the two engine mounts which are toward the passenger compartment and toward the radiator? (the "left" and "right" mounts)?

Other than the front and top and left and right, are there any other engine mounts which usually need replacement?

I'm not particularly concerned with the subframe mounts, but the mounts in the front and rear of the engine bay.

The front one (transmission to engine mount) is easy...it just unbolts and rebolts back on. The one at the rear (assume you're talking about the one under the timing belt) is a bit more involved... you have to peel back the wheel liner, jack up from under the oil pan (use a 2X4) about an inch and you can see if it's broken....then there are several difficult bolts to get out (as they are often seized) in order to replace that mount. It's doable though.
