Rotors generally have 'lines' on them after they have been used a few times but there shouldn't be any deep groves or anything. A pic will be most helpful.

I replaced the shims today and had failed to notice that the wheel that was squealing the loudest does not have the centering screw that goes through the rotor. The rotor has a little wobble with only the caliper, this has to be the culprit. I am going to order one tomorrow.

Even without the 10mm pin-nut thingy, you'll be able to keep the rotor true by taking time to tighten the lugs. Tighten by hand in the regular star pattern, then tighten with a torque wrench to 81 lbs. That will sandwich the rotor between wheel and hub quite well.
