Those symptoms along with rpm going up & down are well known symptoms of a bad maf sensor.That smoke from the exhaust(black smoke) means that the car is running very rich.Maybe the smoke disappears when the engine is warm but the rich smell should be still present.
Disconnect the maf sensor and start the car...if it's bad the engine should run better.
