On Range Rovers and BMW it is a very common issue. Exhaust manifold, cracked cat, and loose spark plug are always recommended places to start. Take a hose to ear and move around the areas.

One question on your car - Volvo switched to solid lifters at one point. My 2001 engine had solid lifters and a single VVT. Can anyone in the "know" provide a more precise cut off date? Would be helpful for future searches.

Aphx: Did you try using a transmission funnel as a ear trumpet to isolate the sound? You can smoke test an exhaust system just as one does with the evap system, just blow smoke up its waste pipe and see where it comes out.

VVT started on 1999 MY turbos and 2000 MY NAs. The easy way to tell if the car has solid lifters is to remove the spark plug cover and look for several torx head screws positioned on the front and rear edges of the cam cover. There will be 20 of them, one for each valve.
If you search around on Volvospeed, there might be some info on VVT/solid lifter cutoff dates or engine models.
That sounds nasty. At first it sounds like an exhaust leak, but when you rev it up a little bit it sounds like it could be lifters. What kind of oil are you using? I have found heavier oils (10w30) make the lifters a little noisier. I doubt that's a problem if it just started happening, though.
