Warping - meaning that the exhaust manifold could be bent or damaged, cracked perhaps?
Its so hard to see and I would need to have it on the lift to be able to look at it from below, to perhaps, see the exact location of the smoke coming out.

Have you been driving with the car for almost 6 years like this?

Yes!

The ECM-4801 is illusive during summer -- ambient temp above 90ºF. The ECM-4810 is present as I write this note, and the mpg has gradually dropped from 20 in summer to 17.8 yesterday.

I've also observed ECM-4801 absent during the road trips -- highway speed driving, i.e. I'll erase/reset before the road trip. Only when I'm in the urban driving conditions with start/stop, it will register at 1600-2000 RPM at 50-60MPH and decelerating -- source: FREEZE FRAME DATA.

Other issues creep up, and schedule maintenance takes priority, for example the 200,000 Miles Makeover viewtopic.php?f=9&t=92790.

It passes State Air Team standard.

We got this car - or were more or less forced to take this car - really cheap.
I hade to change the heater core.... Hope you never have to!!!! It was a pain and in the end, I hade to take out the steering column to be able to push in the pipes to the core. Nasty work - that cannot be done with one pair of hands!!
The new owner had a Ford Focus from 2001 that was not worth fixing - just too much of rust, and what we could do for her, was to put this V70 together so she would have a car.
It has been abused by the previous owner!!! And by the "mechanics" servicing it - a school. We couldnt get the oil drain plug loose! The gliding pins on the callipers were filled with copper paste - that get hard and stiff in the winter weather, so the rear brakes didnt release!!! We use molybdenum fat or silicone.
So - I do not have the time to check out the leak before she needs the car. But will take the opportunity next time it comes in for service or work, and do the smoke detection on a lift and hopefully come back to update here.
