00 s80 t6 problem

i have a 2000 s80 t6.

i seem to be changing my plugs every couple months. car is barely used. when pulling the plugs out they are black with carbon. once cleaned up they go back in and it runs like they are brand new.

i have a brand new cone air filter on, i have recently changed all the coils. could this be due to the o2 sensor being bad or a blocked pcv?

Could be that the mixture is somewhat rich. Is it throwing any codes?

it keeps throwing the codes when the spark plugs go bad and it is for cylinder 3, 4, and 5. these were the last 3 that the coils got changed on. it also throws 2 more but off hand i am not to sure what they are.
