How long should a complete "drive cycle" take in order to run all ODB-II monitors on a 1998 V70 AWD. After an error code reset was performed with an OBD-II scan tool I have driven over 300 miles and at least one monitor "Catalyst" has not run yet. Is that normal for this model? The other four monitors (HC, EV, O, OH) required driving between 150 - 250 miles before running. This seems excessive driving mileage to complete a diagnostic drive cycle.
If all diagnostic monitors have not run successfully does the ECM operate the engine in open or closed loop?

Drive cycle is not just mileage but also includes COLD starts.
