I am having problems getting the cam cover off of my 5254T engine. I have removed all 40 bolts plus the small bolt on the tail of the exhaust cam. The cover is lose on cylinder #5 side and in the middle. It will not come lose over both cams.
What else do I have to do?

Did you first release and remove the timing belt at the #1 side? It sounds like you have removed all of the bolts, there aren't any hidden ones.
The cams need to come up and out of the head as the cover comes up. The whole unit needs to come straight up to avoid cracking the little slots in the head. Is the seal just not breaking loose and the cover is stuck down at the cam sprocket end?
If the cover and/or cams starts coming up cockeyed, for example it is common for the distributor end of the cams to pop up but the sprocket ends to stay in the head, then it is fine to rotate it back and forth along the long axis (toward the intake then toward the exhaust etc.) but not left to right, that will crack the slots. Or you can lever the cams up via the sprockets and a heavy screwdriver.
Take care to not mar the sealing surfaces.
