The cam cover is a used one from same year, model, etc. Clean, engine appeared well taken care of. Decided to take a shot at this due to loosing oil from the fill tube, (it had for some reason come loose in mine and I was loosing a lot of out. It appears to be working ok and I have good oil pressure.
Let me update you please.
I did remove the timing solenoids and checked and cleaned them. they are new, I have replaced them due to all of this and codes. I have much less hair over this. I have, we have, checked timing 3 times now. We do have a cam locking tool set and set the timing according to Volvo.
I went out to the local pick n pull yard, they just got in a 2004 XC90 with a 2.9 T6 engine. It was a good one to look at since it appears to have just had a new timing belt installed. timing marks all made, nice job. Took lots of photos. I was going to pull and purchase the hubs but did not do so. My hubs do not leak any oil.
Came back and decided to dig further.
Now, maybe we found it! the problem. My nightmare all these months. MAYBE!
Opened up the wire harness at the plug to the exhaust solenoid. A wire to the solenoid was found to be loose in the plug. That wire was making intermittent contact with the other now bare wire.
Went back to the salvage yard. Obtain a section of harness, Spliced the new harness. Pull all back together.
Check engine light out!
Going to clear codes and drive today then run Vida again and check for any codes.
Will let you know.
It appears wire became loose in plug then was pulling out.

As I responded to a message yesterday;
I scratched my head and pulled my hair to where it is getting thin and gray...
I pulled the Solenoids the blow out and check the oil passages. I then decided, before I go into the timing again, decided to open the harness engine wiring and start checking that way. Anyway, I found a wire,(s), loose and coming out of the wire harness plug to the exhaust solenoid.. If I could upload pics here I would. went to the salvage pick and pull yard again and obtain that section of harness, after check it also. I sectioned in, soldered, that section of harness.
After driving yesterday, last night, this morning....NO check engine light
No codes!
All these months of frustrations it turns out that a bad wire in a connection has been my nightmare. Loose wires in plugs and connections are not uncommon and I know this. I should have checked the harness wires before and at least eliminated that possibility prior to continuing my frustrations.

Same code, similar issue for my 2007 xc70.
Slightly rough idle and ECM-6140 after doing a timing and PCV job. Spent hours triple checking timing (and confirmed via cam adaption angle in vida), replaced both VVT Solenoids, only to discover an old cracked and corroded pigtail by the connector.
Prancing Moose sells a replacement pigtail.
Volvo Dealership informed me they no longer sell their VVT solenoid wiring connector (exhaust 9441561/ intake 9441560) or wire repair kit (either 30656680 for 0.6-0.75 mm2 or 30656635 for 0-1mm2). You may be able to find a dead stock one somewhere.
Otherwise you’re going to have to scrapyard it. Note that there are a few variations of these connectors, make sure you take photos and match exactly.
Would suggest splicing in your new pigtail as far back (as far away from the hot engine block) as you can.
Check your wires!
