This is the inside of my front rotor...same on both sides. Seems that the pad only connected in the center. They were relatively new pads. I changed the rotors/ pads/ hoses. Just thought I would give you a visual in case you had any input.

Just a guess…… Possibly the guide pins were bound up causing the pads to cock and contact the rotor unevenly?????

I think the rotors warped slightly outwards in the most heated area. This is common, I see it on other vehicles as well, like Honda, Toyota, brand doesn't matter much,

Look at the pads, the disk is showing what the pad did.

what wheels do you have?, good thing about most volvo mag wheels is you can easily see the rotor and the pad.
