2001 S40 Intermittent Engine Surging

Turbo, 116K miles, Second owner, Automatic

This problem has occurred twice in the last week separated by normal behavior:

While slowing down to stop, the engine stumbled/ran rough for a few minutes before stopping. There was a 2 hour delay before tried to restart the car. Upon each subsequent restart, the engine immediately started, rev'd to 2K RPM, then stopped. This behavior was consistent until I also applied enough throttle to keep the revs above 2K RPM. With the engine at 2K RPM for several seconds I slowly reduced the throttle over 30 seconds or so after which he engine idled smoothly. I drove it home with no notable behaviors to report. It drove fine for another 40 miles over the next two days before the second engine failure. This time it was more difficult to keep the engine running, but I eventually limped home, where in Park with no throttle depression, the engine ran smoothly but surged between 1K and 2K RPMs until I turned it off. I read the following codes all of which, save P0131, I have not seen before:
P0131 - O2 Sensor Circuit Low Voltage Bank 1, Sensor 1 (this has been a recurring issue)
P0301 - Cylinder 1 Misfire Detected
P0302 - Cylinder 2 Misfire Detected
P0303 - Cylinder 3 Misfire Detected
P0304 - Cylinder 4 Misfire Detected
P0401 - Exhaust Gas Re-circulation Flow Insufficient

hi i know its been a while since you posted this but did you ever figure it out. im having this exact issue withone of my cars but i cant find anything on the internet

Given those codes,I would guess a coil issue.
'60 PV544, '68 220, '70 145S, '86 745T, '95 854T, '01 S40
