Seems like a crazy case of bad luck. (And who among us is worthy of such bad luck?) I would take apart the PCV again, piece by piece, and make sure there's no funny business in there. Maybe you did disconnect and connect something erroneously... I'm with polskamafia, I don't think a "clear" PCV could've caused this.
About the consumption of coolant. There quite a few reasons as to why that could have occurring, e.g., heater core, system burping, slow leaks here and there that only occur when driving, faulty reservoir cap, wrong coolant solution... Take everything apart again and double and triple check.
I'm hopeful. But I'm always hopeful! Good luck figuring this out.

Independent failures. Figure out if it's oil from engine or ATF and then source accordingly.

The oil cooler is on the left (passenger side) of the radiator. There are connections at the top and bottom that you will see.JasonVolvoMan wrote:Where is the oil cooler located

The headgasket and radiator are the only two places where coolant and oil can mix.polskamafia mjl wrote:...the oil cooler in the radiator...

Random failures that seem to be related but they're not- I've had them on my Volvo's....usually happen in threes not in pairs. It's just bad luck I think. In your case, I hope you can figure it out. On my son's 850 we had a transmission go bad because the radiator breached between the coolant and transmission fluid. In your case, I'm hoping it's your radiator and not your head gasket. It's a hassle but much cheaper and easier to deal with and at least you found it early...early enough that if that's what it is, your engine is probably okay- you'll just have to drain and fill with oil- maybe add something to clean up the breach... or do another oil change fairly early after things are cleared up.
