Glad you got to the bottom of it. I'm fairly sure we have all done that repair that didn't quite go to plan, or forgot to tighten something. My next suggestion was, did you change the CBV diaphragm when the turbo was repaired.

Beats a clogged cat anyday. Good it was something simple. A bent nail or even a paper clip will keep it in place until you can get a cotter pin.
