I am having the exact same problem with my 2001 s60 volvo. The power steering fluid dumps out while driving. The nut on the pressure line keeps loosening itself off while driving. Ive put in a new oring that hasnt seemed to do the trick. The only thing i dion't have that i see in your pictures is the bracket you have pictured. Is it possible it may have jiggled itself off while driving and I lost it? My thinking is that without that bracket it is causing the connection to work its way off the pump with ever bump on the road i encounter.

So I am having the exact same issue, at least in the area of the leak, but have replaced (more than once to ensure I didn’t do a bad install the first go around) the O ring and done lock to lock test and it is still leaking from the return connection, OP, can you share what was the final solution?
I had the power steering rack fully replaced and then a leak cropped up. Thought i resolved it by replacing the clamps and O ring, drove 500 miles no issue and then parked for a day. Now I’ve driven it a short trip and leaking. Not sure what to do at this point since the leak is from the return connection on the pipe side of the connection. Any help is appreciated!
