2000 V70 Cracked Injectors

Pulled the fuel rail to reveal a chunk of injector missing at the o-ring. This is the second time this has happened. Are these injectors poorly designed/manufactured or could something inside the combustion chamber or intake be causing this? Also - seems to run fine. Any harm in just leaving it as is?
