I was watching a YouTube video that showed an Intake Banjo bolt that failed and destroyed the engine. I've never heard of this before. Searches find posts about installing the banjo bolt after completing the PVC job because it's a pain to get started.
Have others heard of this and any idea how high is the failure rate? Is it possible the root cause of some engine failures went unresolved because of this strange failure. It does seem like Volvo redesigned the bolt at some point.

There is a failure mode where the check valve in the banjo bolt loses the pin and ball and it gets whipped through the engine.
Not all banjos have this check valve, the late P80 and early P2 don’t have it
