Our black 07 XC70 is currently (still) laid up until I pull the top apart, but all indications are that the little piece got pulled in to #3 and is bouncing around on top.

Onto the side of the AC compressor. If you look at your 2nd picture, and look straight down from the connector you can see a little shiny metal pin that looks like it is at the side of the lower part of the alternator.

I was having similar issues on my '06 and opted to buy a clean, used ETA from the local wrecker for $25. Before mounting it on the car I took the plastic cover off of the side to confirm it had no oil intrusion and that the plastic gear teeth were still intact. That solved my issue.

This is typical failure. If you plan to keep this vehicle a long time, consider replacing the entire fuel pump while you have it open. Then you have no fuel pump worries for a long, long time.
