depends on the make - Hidden Hitch, Reese, Dalan, etc. and the rating; probably Class II - but it is likely true that only 4 bolts hold it on.

The Hidden Hitch model I'm putting on my 98 V70 T5 has only 3 bolts per side.

With a 96, you don't have the old method of accessing the trouble codes at the front of the vehicle. The ECC codes are not standard OBD2 and thus won't show up with a standard code reader. Need to get a dealer to pull them with a Vadis system.

If you're going into the shop, it'd be worth having the mechanic do a quick diagnosis (pull the codes with the VADIS tool) and decide whether to have them fix it or DIY.
