Did you do any code reads on this vehicle?
Fan is very variable, stops working, works, speeds up, slows down, all a function of what the ECM tells it to do via a signal (PWM).
ECM is looking at engine coolant temperature (ECT) sensor as well as the vehicle speed to tell the fan the speed to run ...

Has someone removed the carpet floors under the front footwells and validated that the smell is not coming from the structural foam floor or below it? Not to be confused with carpets being wet, this is the residual smell and the foam itself will need cleaned and treated.
