Misfire codes take a while to set, so don't put a lot of value in their absence. Likewise, fuel condition fault programming is complex

I took the oil cap off the other day when the engine was warm and notice a little bit of white smoke coming out. Could this be a bad pcv valve causing this and therefore causing it run rich?

A bit of smoke is ok, it wasnt chuffing like a locomotive, was it?

No it was not chuffing, just a tiny bit of white smoke. Almost like when you were to light a match or something... not a lot.

Did you ever figure out what was going on? I'm having the same exact problem with my 2004 XC70, p2188, worse after parking with car facing downhill. I replaced the MAF, and it went away for about 2 weeks then came back. And if I disconnect the battery the problem goes away for a couple hours then starts up again.

No, I have not figured it out. I replaced all the leaky vacuum hoses, it idles smoother and less rough. But I still get the running rich code, and then car surges on start up a few times until it levels out. If I clear the code, the car will start fine until the code sets, and then it will have issues upon starting. I can't figure it out.
