I forgot to mention the PTC nipple. It attaches into the hose that runs from the MAF sensor to the turbo.
I think that can get pretty oil-fouled over the years and may no longer make a tight seal. Hence you'd have a vacuum leak there.

I just did a vacuum test, and I also have a pretty big vacuum leak, I further found out that my intake manifold gasket had slipped down when I installed it.
Everything is good now, starts right up

I replaced the IAC unit and the problem has resolved. I bought a cheap aftermarket one since the Volvo one is 150$. Kept my old one just in case, but it seems fine now.
