Yes and yes.

Our black 07 XC70 is currently (still) laid up until I pull the top apart, but all indications are that the little piece got pulled in to #3 and is bouncing around on top.

A little lazy to research and read so appreciate if you can tell me which years have the bad banjo bolt? Currently in the fleet.

I don't know! And you'll find conflicting info online. However consider that the go-to Pine Hollow videos (see post up-thread by jonesg) are dealing with an 08 S60, to me that says clearly that all P2 are suspect, and probably beyond. I'd take a look at the bolt on your 09.

Nice job. Yesterday I tried to get a light and a mirror close to it. All I can see is the side of the bolt head, so it's still a mystery.

I think they all had the failure prone one originally until an update became available about 5-8 years ago. FCP should be able to provide a part number or a call to the local or nearest Volvo dealer.

This forum posting for a disassembled PCV check valve banjo bolt was key for me to figure out what happened to my 2007 Ti S60 that had bad compression on cylinder 3.

After a bunch of research, I opened the engine and found damage from the pin and ball:

I purchased a replacement head at a pick-n-pull junkyard along with a like new PCV system. The valves were all cleaned, seats and valves lapped, valve stem seals replaced, and clearances verified to all be in tolerance for the solid tappets.

The protrusions on the piston were all smoothed.
Then I learned that the PCV valve guts had also gone through to the turbo:

I got a turbo out of a low mileage junkyard car and rebuild it with a kit recommended on this site. (Soaking in strong phosphoric acid does a great job removing the buildup of exhaust contaminants on the impeller.)

I temporarily used a PCV check valve from the junkyard car and then replaced it with the new design check valve. Here is a complete lineup of the 4 Volvo P2 platform banjo bolt versions that you will encounter:

The information on this website plus Vida/Dice, junkyard parts, FCP Euro parts, and a bunch of new tools and time got my S60 back on the road.
I hope this post helps some other Volvo owners get the message that replacing your PCV banjo bolt is a very good idea!

How many hours of work to replace the banjo bolt? Looks like access is removing the steering pump and removing with ratcheting box end wrench?

Asking as I know the PCV job is quite a chore.

Need to unbolt alternator and shift it to left as well.
