The locking ring should hold it in place, but yes it is suppose to look like the old one. If you couldn't yank off the axle then it should be good. It's really all opinions. You'd already change a majority of the fluids only for the next round to be less than that. I only changed it once and never bothered doing another round.
I'd say you either do it back to back or whenever you get to it. Maybe just do it with your next oil change.

Date should be on the boot. It looks like a clock with a ring and arrow; arrow pointing to month and year is between the arrow.

No, its almost always a tiny PCV hose under the intake manifold that needs to be replaced. The PCV job is the most intense DIY so you'd want to look into that, and to do the whole system is about $400 for parts and another $450 for labor for an indie last I checked.
